By submitting this form you are agreeing that you are happy for your child:
a) To take part in school trips and other activities that take place off school premises;
b) To be given first aid or urgent medical treatment during any school trip or activity and
c) To be carried by public transport or school transport driven in a responsible manner by an adult who is suitably qualified and insured.
d) For up to £35.00 to be added to your bill to cover any costs involved
Please note the following important information is included in the consent when you submit this form
- The trips and activities covered by this consent include;
- all visits (including residential trips) which take place during the holidays or a weekend
- adventure activities at any time
- off-site sporting fixtures outside the school day
- boarders weekend activities
- The school will send you information about each trip or activity before it takes place.
- Your prior consent will be sought for a trip incurring costs exceeding £35.00.
- You can, if you wish, tell the school that you do not want your child to take part in any particular school trip or activity.
- Your child is subject to School discipline in all respects whilst engaged in a School trip.
Written parental consent
will not be requested from you for the majority of off-site activities offered
by the school – for example, year-group visits to local amenities – as such
activities are part of the school’s curriculum and usually take place during
the normal school day.
For residential visits, you will be sent a medical and code of conduct consent form via EVOLVE (our trips planning platform) to complete and submit in order for your child to take part in the residential visit.
