Please complete the forms by clicking on the links below and completing each form that is relevant to your child.
Please be aware that each individual form will need to be completed in one go.
Forms 1, 2 and 3 MUST be completed by the given deadline.
1. Pupil Details Form - this form requires: details of the pupil; parent addresses and phone numbers; emergency contact details and a copy of the pupil's passport.
2. Medical History Form - This form requires: immunisation details; details of medical conditions, allergies and medication; and includes consent for treatment.
3. Trips Consent - This form gives permission for the pupil to take part in trips away from the School site.
4. Nursery Sessions Form (if applicable) - This form is to book Nursery sessions.
5. Instrumental Lessons - In this form you can select which instrument you would like to learn and whether you need to hire an instrument.
6. Bus Transport Information - this page contains information about the School's bus transport.
